This is the Digitech Whammy Wah XP-100 Electric Guitar Effect Pedal.
The Whammy took the world by storm in the late 80's- it became a large part of some famous player's signature tone- from Dimebag to Billy Corgan.
The XP-100 took it a step further by adding wah effects to the mix.
There are pitch shift effects as well as wahs, auto wahs, and harmony.
This pedal is in good overall condition as you can see in pics- typical marks for a 20+ year old effect.
